
| Deadline: | Pre-registration must be postmarked by Friday, October 14, 2011. Registrations postmarked after October 14, 2011 add an additional $20. All those pre-registered will receive an e-mail confirmation. |
| Payments: | All payments, including purchase orders, must accompany registration form or the registration P.O. or checks made payable to AGATE |
| Cancellations: | For cancellations received in writing before October 21, 2011, full registration amount paid will be refunded – less a $25 processing fee. No refunds after October 21, 2011. |
| On-Site Registration: | Forms will be available. Add $20.00 to registration fee. |
| Hotels: | Information will be emailed with registration confirmation |
