Deadline: Pre-registration must be postmarked by Friday, October 14, 2011.
Registrations postmarked after October 14, 2011 add an additional $20.
All those pre-registered will receive an e-mail confirmation.
Payments:

All payments, including purchase orders, must accompany registration form or the registration
form will be returned to you.

Cancellations: For cancellations received in writing before October 21, 2011, full registration amount paid
will be refunded – less a $25 processing fee. No refunds after October 21, 2011.
On-Site Registration: Forms will be available. Add $20.00 to registration fee.
Hotels: Information will be emailed with registration confirmation

Registration includes Friday appetizer reception with live entertainment, Saturday continental breakfast, and lunch.
